COVID-19 Safety Acknowledgement Liability Waiver and Release of Claims

COVID-19 SAFETY INFORMATION

[When Applicable] While participating in events held or sponsored by the New Jersey chapter of Community Associations Institute (CAI-NJ), “social distancing” must be practiced, and face coverings are always worn to reduce the risks of exposure to COVID-19. Because COVID-19 is extremely contagious and is spread mainly from person-to-person contact, CAI-NJ has put in place preventative measures to reduce the spread of COVID-19. However, CAI-NJ cannot guarantee that its participants, volunteers, or others in attendance will not become infected with COVID-19. Further, prior to attending any CAI-NJ event, participants must review the CAI-NJ Event Reopening Guidelines, as published on the CAI-NJ website.

In light of the ongoing spread of COVID-19, individuals who fall within any of the categories below should not attend or engage in CAI-NJ events. By attending this CAI-NJ event, you certify that you do not fall into any of the following categories within 14 days of the event, otherwise, you will not attend:  

  1. Individuals who within fourteen (14) days of the event have experienced any symptoms associated with COVID-19, which include fever, cough, and shortness of breath among others;
  2. Individuals who have traveled at any point within the past fourteen (14) days either internationally or to a community in the U.S. that has experienced or is experiencing sustained community spread of COVID-19; or
  3. Individuals who believe that they may have been exposed to a confirmed or suspected case of COVID-19 or have been diagnosed with COVID-19 and are not yet cleared as non-contagious by state or local public health authorities or the health care team responsible for their treatment.

SAFETY GUIDELINES NON-COMPLIANCE

Failure to comply with CAI-NJ, the State of New Jersey, or Federal health guidelines (if applicable) may result as follows:

  1. If my temperature, if taken before entering a CAI-NJ event, is above the safe range, I will not be permitted to attend the event; and/or
  2. If I fail to comply with the CAI-NJ Event Reopening Guidelines, it will result in my removal from the event and potentially being banned from future CAI-NJ events as determined by the CAI-NJ Board of Directors.
  3. If I fail to comply with safety precautions, such as face coverings or temperature checks, it will result in my removal from the event and potentially being banned from future CAI-NJ events as determined by the CAI-NJ Board of Directors.

EVENT PARTICIPANT RELEASE AND WAIVER

I understand that participation in this event carries with it risk of injury or death and I, on behalf of myself, my heirs, administrators, executors, and assigns expressly release, forever discharge, and hold harmless CAI-NJ, the CAI-NJ Board of Directors and CAI-NJ employees (the “Releasees”) for all personal injury, loss, damage to property, or death in connection with my participation in this event. Further, I understand and acknowledge that there currently exists a global pandemic associated with the disease COVID-19, that I am participating in this event at my own risk, that any safety precautions designed to protect against the spread of COVID-19 undertaken by the Releasees does not guarantee I will not contract COVID-19, and that, the foregoing notwithstanding, it is my specific intent to affirm my foregoing release of the Releasees with respect to any personal injury, loss, damage to property, or death I may experience from COVID-19 in connection with my participation in this event.

If you are attending more than one event, you must fill out and submit this form for each. Only one attendee per submission. This DOES NOT serve as your registration for the event selected. You must still register via the event page in order to be fully registered to attend.
